congratulations on the award! i know you love volunteering for them and it's really kewl that they acknowledged your efforts :) yay!
as for work stuff.... are you burned out? maybe you need to take a couple days off and rest and regroup? or is it that you just have so much to do that you don't have time to make sure all the details are correct? maybe have a talk with dave about it? i know you said when he got back from his sabbatical that some of his stuff was permanently transitioned to you, which means you might have more on your plate than you can handle. if you're swamped and you've finished a report, could you ask someone to just take a look at it before you turn it in and make sure you didn't miss anything?
